Camp Crossroads

Camp Crossroads is hosted by Camp Elim in Woodland Park, Colorado Springs, Colorado.  Below are the health/safety forms and registration forms required by Camp Elim, as well as the physical form.  Please fill out these forms and bring them with you the day of camp.  Camp Crossroads counselors will collect these forms.  The physical form may be less than 2 years old.

A state-licensed physician will be present the day of camp at the Chinese Evangelical Church of Denver (CECD).  If your youth needs a last minute physical, he/she may do so the day of camp at CECD for $20/physical.

Please be sure each camper brings all of the appropriate forms as these are required by the state in order for your youths to attend Camp Crossroads at Camp Elim.
